This pattern arises from stagnant qi, often due to irregular eating, emotional stress, or extreme environmental factors, which disrupt the normal flow of energy. When qi stagnates in the middle burner, it causes focal distention and a stifling sensation in the chest and abdomen, along with digestive disturbances such as bloating, reduced appetite, indigestion, nausea, vomiting, and mild coughing with copious sputum. Stagnant qi may transform into fire, leading to acid reflux and a bitter taste in the mouth, or into blood stasis, causing fixed, stabbing pain in the hypochondria. The underlying problem is a failure of qi to ascend, descend, or transform properly, resulting in a variety of physical and digestive symptoms.

The chief herbs are Atractylodis Rhizoma (cang zhu), which moves qi, dries dampness, and transforms phlegm to relieve chest distention and copious sputum, and Chuanxiong Rhizoma (chuan xiong), which invigorates blood to resolve fixed pain and reinforce qi movement. The deputy, Cyperi Rhizoma (xiang fu), balances upward- and downward-directing qi, complementing the chief herbs and addressing gynecological issues. Assistants include Gardeniae Fructus (zhi zi), which clears heat and fire from constraint, and Massa medicata fermentata (shen qu), which harmonizes the stomach and alleviates nausea, vomiting, reduced appetite, and epigastric stifling. Together, the herbs restore the proper flow of qi and blood.
