Li Hong Tang (also known as Juan Bi Tang in some classical lineages) is the premier formula for Upper Body Bi-Syndrome. While many formulas treat general joint pain, this specific combination is surgically targeted at the neck, shoulders, and arms, particularly when the pain is caused by a "leak" in the body's protective Qi that allows Wind, Cold, and Damp to settle into the joints.
Symptoms
Upper Body Pain: Chronic aching, stiffness, or sharp pain specifically in the neck, shoulders, and upper back.
Numbness & Heaviness: A sensation of "dead weight" in the arms or tingling in the fingers (numbness due to blood and Qi not reaching the extremities).
Limited Range of Motion: Difficulty turning the neck or lifting the arms (Frozen Shoulder / Adhesive Capsulitis).
Weather Sensitivity: Pain that worsens with wind or damp/rainy weather.
Key Indicator: "Deficiency-Precipitated Bi Syndrome"—where the pain persists not because the pathogen is "strong," but because the body’s Qi and Blood are too "thin" to push the wind and dampness out of the channels.
Tongue: Pale or pink with a thin white coating.
Pulse: Moderate (Huan) or Thin and Wiry (Xi Xuan).

Description
The technical brilliance of this formula lies in its "Double-Door" strategy: it opens the "exterior door" to let the wind out while simultaneously reinforcing the "interior walls" so the wind cannot return.
The Moving Pair (Jiang Huang & Qiang Huo): These are the "Lead" herbs. Jiang Huang (Turmeric) breaks the physical stagnation in the shoulder joint, while Qiang Huo provides the upward-moving momentum to clear the neck and upper back.
The Shield (Huang Qi & Fang Feng): This combination creates the "Jade Screen" effect. Huang Qi provides the pressure needed to expel the dampness, while Fang Feng ensures the surface remains sealed against new environmental invasions.
The Blood Support (Dang Gui & Bai Shao): Since "to treat Wind, one must first treat the Blood," Dang Gui moves and tonifies the blood, while Bai Shao softens the tendons. This "lubricates" the joints, allowing for a restored range of motion.
Posology
Granules
3 to 9g daily, divided into 2 doses, taken with warm water after meals.
Raw Plants
Decoct in 800ml of water and drink 1 to 2 cups per day. For acute stiffness, the decoction should be taken warm to encourage mild sweating, which helps "vent" the wind.
Cautions
Not indicated for Heat-Bi. Avoid if joints are red, swollen, and hot to the touch; this formula is warm and acrid and will worsen inflammatory heat. Use with caution in cases of severe Yin deficiency (dryness/night sweats). Because it contains strong blood-movers like Jiang Huang and Dang Gui, consult a practitioner if taking blood-thinning medications.
