Symptoms

Cold Extremities: Long-standing cold hands and feet that feel icy to the touch, often extending up to the wrists and ankles, worsening in cold weather.

Pain & Discomfort: Body aches, joint pain, or cold-induced cramping in the limbs, hands, or lower abdomen that improves with heat and worsens with exposure to cold.

Complexion & Circulation: Pale, dull, or purplish/cyanotic fingernails and lips due to poor peripheral circulation and blood deficiency.

Gynaecological Signs (if applicable): Painful, delayed menstruation with dark blood or small clots, aggravated by exposure to cold.

Tongue: Pale body with a thin, white coating.

Pulse: Deep (Chen), thready (Xi), and almost imperceptible, or thin and wiry (Xian).

Posology

Granules: 3 to 9g per day, divided into two or three doses.

Raw Herbs: Boil in 1000–1200ml of water and drink 1 to 3 cups per day.

Description

Dang Gui Si Ni Tang (Tangguei Decoction for Frigid Extremities) is the quintessential classical formula for treating cold limbs caused by Blood Deficiency failing to fill the vessels combined with Cold Pathogens obstructing the meridians.

Dang Gui serves as the Chief herb; it intensely nourishes the Blood, invigorates circulation, and directly addresses the core deficiency that prevents warmth from reaching the hands and feet.

Gui Zhi and Bai Shao form the foundational pair of the formula. Gui Zhi warms the channels, unblocks the blood vessels, and disperses cold, while Bai Shao nourishes the Blood and softens the Liver, preventing Gui Zhi from over-drying the body's fluids.

Mu Tong plays a crucial role in clearing and unblocking; it drains the channels, opens the micro-vessels, and ensures that warm Blood can flow all the way to the fingertips and toes.

Da Zao and Zhi Gan Cao tonify the Middle Jiao Qi, generate new Blood, and harmonize the active warming herbs to ensure sustained, lasting therapeutic effect.

Contraindications

STRICTLY CONTRAINDICATED for cold extremities caused by true Interior Heat with false cold signs (Yang collapse or constrained heat patterns).

Do not use in cases of Yin Deficiency with Empty Heat (red tongue without coating, night sweats).

Use caution during pregnancy due to the blood-invigorating and channel-opening properties of Mu Tong.
