Symptoms:
The "Four Bigs": Big fever (high fever), Big sweat (profuse), Big thirst (unquenchable), and a Big pulse
Severe irritability and restlessness
Mental confusion or slight delirium in extreme heat cases
Aversion to heat and a desire for cold drinks
Key Indicator: Significant physical exhaustion or "wilting" following a high fever, indicating that the heat has begun to scorch the body’s fluids and Qi
Red tongue with a dry, yellow coating or a dry, parched tongue; Pulse is large, forceful, and surging, but may become weak upon deeper pressure (indicating Qi deficiency)

Posology
Granules: 3 to 9g per day after main meals, diluted in hot water.
Raw Plants: Boil in 1000 or 1200ml of water and drink 1 to 3 cups per day. (Note: Shi Gao should be crushed and pre-boiled for 20 minutes before adding other herbs).
Description
Bai Hu Jia Ren Shen Tang, known as "White Tiger plus Ginseng Decoction," is a powerful clinical evolution of the standard Bai Hu Tang. It is designed to address a critical stage of febrile illness where extreme Heat in the Qi level is simultaneously destroying the body’s Fluids (Yin) and depleting its functional Energy (Qi).
In Traditional Chinese Medicine, when Heat is "blazing," it acts like a fire that evaporates the water in a pot and eventually burns the pot itself. This formula acts as a "fire extinguisher" that also refills the water and repairs the vessel.
Shi Gao (Gypsum fibrosum) is the Chief herb, a mineral with a very cold nature that clears massive heat from the Lungs and Stomach. It is unsurpassed in its ability to drop a high fever and stop the "Surging" pulse.
Zhi Mu (Anemarrhenae Rhizoma) acts as the Deputy, assisting Shi Gao in clearing heat while specifically moistening dryness and nourishing the Kidneys and Lungs to protect the body's fluid reserves.
Ren Shen (Ginseng Radix) is the critical addition to the "White Tiger" base. It targets the "Big Pulse" that has become hollow or weak. It tonifies the Yuan Qi and generates fluids, ensuring the patient does not collapse from exhaustion caused by the profuse sweating and high fever.
Zhi Gan Cao (Glycyrrhizae Radix preparata) and Geng Mi (Nonglutinous Rice) work together to protect the Stomach and Spleen. Because the primary herbs are extremely cold, these two ingredients prevent the "fire-fighting" herbs from damaging the digestive fire, while also generating fluids to soothe the parched throat and tongue.
Together, these five ingredients create a formula that clears heat without causing further dryness and tonifies Qi without trapping the heat. It is a masterclass in balancing aggressive clearing with gentle nourishment.
Contraindications
Strictly contraindicated for fevers caused by exterior Wind-Cold (where there is no sweating or the pulse is tight). Not for use in cases of "False Heat/True Cold" or Spleen-Deficiency cold. Use with caution in patients with very weak digestion. Consult a practitioner before use.
