This pattern represents a combination of lesser yang (shaoyang) and yang brightness (yangming) disorders. Symptoms such as alternating fever and chills, chest and hypochondriac fullness, bitter taste, nausea, and wiry pulse indicate a shaoyang disorder, while epigastric distention or pain, constipation or burning diarrhea, yellow tongue coating, and forceful pulse reflect yangming involvement. The pathogen has penetrated deeper, leading to more intense heat and constraint, which explains vomiting, irritability, and melancholy.

The treatment focuses on harmonizing the shaoyang and clearing internal heat from the yangming to restore proper qi flow. The formula can also treat mixed heat with diarrhea, a condition marked by internal heat and disturbed qi dynamics, though without the strong tongue and pulse signs typical of full yangming heat.
